We present a clinical case of infective endocarditis in which three major complications are developed at the same time, a cardiac one (severe mitral regurgitation), and two extracardiac ones: low limb embolic acute arterial occlusion and splenic abscess. We also make a brief discussion updating the therapeutic of the complications.
