Kounis syndrome is an underdiagnosed entity with few cases published in our country. The diagnosis is clinical, with symptoms and signs suggestive of an acute allergic reaction and an acute coronary event, and must have a temporal cause-effect relationship. The diagnosis is supported by the electrocardiogram, myocardial damage markers and parameters that show an allergic reaction. The treatment is controversial based on 2 pillars: treatment of the allergic reaction and the coronary event. We present the clinical case of Kounis syndrome type I secondary to bee stings that presented a good evolution with antiallergic treatment.
