The persistent increase in cesarean deliveries as a means of completing pregnancy represents a critical public health issue worldwide. This has been highlighted by academia, international health institutions, and feminist studies. In Uruguay, 63.81% of births in the private sector and 36.19% in the public sector in 2023 were by cesarean section. Cesarean delivery on maternal request (CDMR) is one of the main causes of this increase, being considered a non-medically indicated intervention that may pose unnecessary risks to maternal-fetal health. From a principlist perspective, the ethical conflict between the principles of autonomy and non-maleficence is identified. This article explores the analysis of CDMR from a feminist bioethics perspective, proposing the notion of relational autonomy and shared decision-making as ethical frameworks to address this conflict and ensure the sexual and reproductive rights of pregnant individuals.
