Hydatid cyst continues to be a public health problem in Uruguay despite efforts made in primary and secondary prevention, and in fact, its incidence is high.

Hydatid cyst of the soft tissues is rather unusual. Estimated incidence ranges between 0.5 and 5.3%.

The study aims to present the clinical case of a patient carrier of bone marrow compression caused by a complicated lumbar hydatid cyst, something significantly rare. Liver (75%), lungs (15%), brain (2-4%), genitourinary tract (2-3%) are the most frequent sites where hydatid cyst is found. Spinal cord compromise occurs in less than 1%. Compression symptoms are not frequent, although it is one of the potential complications, what resulted in the clinical case being published.
