Summary Objective: to describe clinical and presentation characteristics of a cohort of patients with chronic ulcerative colitis (CUC). Methods: a cohort of 121 patients was retrospective and prospectively analyzed. Diagnosis was determined in patients of urban and rural Uruguay from both public and private health centers between 1951 and 2003 and seen by the authors at least once before June 30, 2003. Clinical histories were used to record age of disease development, topography, severity, extra intestinal manifestations, surgery requirements and mortality. Results: 53.7% were women and 46.3% men. Minimum age of disease development was 3 years and the maximum was 76, the median was 32. Compromise was: 18.8% rectum, 22.2% rectosigmoid, 19.8% left colitis and 38.5% pan colitis. Beginning of the disease was mild in 47.1%, moderate in 31.4% and severe in 18.2% (p=0,0001). Extra intestinal manifestations were seen in 7.4% and family history of CUC in 4.1%. Ten patients (8.3%) required surgery, with a median of 5.5 years from the beginning of the disease to the surgery, 70% due to a peak severity. Mean time of follow up was 10.5 years. Colon cancer was observed in three patients (2.5%). Conclusions: this cohort showed low surgery requirement associated to a peak severity and was performed mainly during the first year of the disease, low incidence of cancer and mortality
